What services does the Council provide?

We provide a full range of services, community support and recreational programs to assist and maintain a person’s independence and quality of life at home and in the local community.

These services provide assistance to the elderly, younger/adult people with disabilities, their carers and families.

Who is eligible?

Services are available to residents of Brimbank who are eligible for the Home and Community Care (HACC) service:

  • Older People and Frail aged Residents
  • People with a Disability and
  • Carers of the above.

What is Home and Community Care?

Home and Community Care(HACC) is a State and Commonwealth Government funded program that provides funding for services to support older people and people with disabilities and their carers.

The aim of this program is to provide basic support and assistance to people living at home who are unable to undertake daily tasks and who wish to continue living independently.

What is the cost of the services?

Details of your service fees will be discussed with you at the time of your assessment.

How am I assessed for the services?

A Council Assessment Officer will meet with you in your own home to look at what support you require and to discuss the type of services/supports available.

Once the assessment has been completed a Care Plan/Care Contract will be developed that provides details of:

  • Type of services and tasks to be provided
  • Duration of service and its frequency and
  • Individual or special service requirements.

The Care Plan/Care Contract will be reviewed on a regular basis to ensure that the service is satisfactory and that it meets your needs

How can I access the services?

Anyone can make a referral or enquire about services. Access to services is based on assessed need. The assessment will determine the type and level of services you need and how often you require.

A range of In-home Support Services for seniors and the aged in Brimbank.

Social Support Programs

Support Programs in Brimbank inculding planned activities groups, recreational programs, carers support for older, aged, and disabled people.

Community Aged Care Packages

Community Aged Care Packages, providing personal and home care, community transport, social support programs and respite care.

Clubs and Groups

We are home to a large number Senior Citizens Clubs/Groups. These groups provide a range of social activities and other support for older people from a wide range of cultural backgrounds.
