Community Aged Care Packages
We are funded by the Commonwealth Department to provide Community Aged Care Packages to eligible frail older and disabled residents of the municipality.
The aim of the program is to maintain the independence of older people and enable them to live in the community for as long as possible.
Who is eligible?
Residents of the City of Brimbank who are assessed as low level care by the Aged Care Assessment Service.
These people require assistance to maintain independence and include those who are:
- Frail aged or have a disability
- Are living alone or are supported by a carer who needs assistance and
- Require co-ordination or management of services.
What services are provided?
- Personal Care
- Home Care
- Community Transport
- Social Support programs and
- Respite Care.
Other services may be available to suit your specific needs. We can develop Individual packages of care for clients and adjust them as necessary to meet changing needs.
What are the costs?
A maximum weekly charge of 17.5% of the aged pension is normally applied; however costs may be negotiated according to financial circumstance.
